Functional Outcomes of Multiligament Reconstruction in Traumatic Knee Injuries: A Case Report

Introduction: Multiligament knee injuries (MLKIs) are uncommon but severe traumatic injuries involving the disruption of two or more major knee ligaments. These injuries are associated with significant instability, impaired knee function, and poor long-term outcomes if inadequately treated. Advances in arthroscopic reconstruction techniques and structured rehabilitation have improved functional recovery.
Aim & Objective: To evaluate the functional outcome of a traumatic multiligament knee injury managed with multiligament knee injury involving the ACL, PCL, and MCL.
Materials & Methods: A 39-year-old male presented after a road traffic accident with severe knee pain and inability to bear weight. Clinical examinations revealed positive anterior drawer, posterior drawer, and valgus stress tests. MRI confirmed complete ACL, PCL, and MCL tears with PCL femoral avulsion. The patient underwent arthroscopic ACL reconstruction using an ipsilateral peroneus longus tendon autograft, PCL avulsion repair, and open MCL reconstruction with a hamstring tendon graft, followed by a standardized rehabilitation protocol.
Results: At the nine-month follow-up, the patient demonstrated excellent functional recovery. He was able to walk independently without pain or instability, had no episodes of knee giving way or clicking, comfortably performed cross-legged sitting and squatting, and achieved a knee range of motion from 0° to 130°.
Conclusion: Early anatomical reconstruction combined with a structured rehabilitation program can result in excellent functional outcomes following multiligament knee injuries. Individualized surgical planning based on the pattern of ligament injury and patient-specific functional demands is essential for restoring knee stability and achieving optimal recovery.
